I think you are just going to have to call.  I didn't have any problems
with any of the times I called Nokia USA, and I just used the phone number
listed on my order.  As Quim indicated, all of the on-line stores should
now be familiar with the coupon deal.  Just give them your order number to
expedite the transaction.
