Is it possible to combine [640x480] (resize-when-loading) with [0]
(first-frame)?

The -size option is passed to the JPG library so it doesn't need to read
in as much of the image.
It will in fact read in about a power of 2 larger than the size given,
but the result is a smaller image. EG -size 640x480 will read in an
image sized to be between 1024x512 to 2048x1024 pixels.
The resize thus has much less work to resize the image to the exact size
given.
The -size is only used for the JPEG image format, and image creator
formats.
Actually with the latest IM versions you can also get IM to resize
images as they are read in (rather after all the images are read in..
This is that save as your command...
convert -size 640x480 very_big_file.jpg'[640x480]' outfile.png
and will work for all image formats. This method only becomes important
when you are montaging multiple images...
montage -size 640x480 'images_*[640x480]' \
-tile 1x1 -set label '%c' -frame 5 -strip \
-set delay 500 -loop 0 commented_image_slideshow.gif
Note the use of -strip to remind any metadata from the image AFTER the
image labels have been set..
