We tried your script with the current release, 6.3.0, and it worked fine.
We turned off dithering:
$image->Quantize(colors=>16,dither=>'false')
Upgraded to 6.3.0 on my local machine (the previous 6.2.8 install)
and indeed things now work as they should.
Bizarrely I did manage to get the expected results in 6.2.8 - by
preceding the Quantize call with a somewhat redundant but magickal
$image->Rotate(degrees=>0)
Now I just have to persuade the maintainer of the 6.2.2 install to
bite the bullet...
