Profile Pictures Default

Bug description:
If in Profile Pictures dialog one clicks to "Use no default" there is
no radiobutton checked (which is correct). If you click "Set Default"
immediately afterwards you get:
Mahara: Invalid Parameter
A required parameter is missing or malformed
Missing parameter 'd' and no default supplied
I think, there should be a proper error message instead. Such an error
message is displayed properly when you try to upload a picture which
is too big.
Mahara Version is 1.4
