Note blocktype and artefact can get out of sync

Bug description:
Editing a note in the notes page causes the note to be saved to the
artefact table only, and not to the block_instance table, and hence
the note artefact becomes "detached" from the block instance
Once this happens, it prevents the notes page from having a complete
collection of notes, and the note can now become different by being
edited in two separate places.
