Input help text should be linked using ARIA

Bug description:
It would be helpful for users of screen readers to be able to hear the help
text assigned to form fields. Setting the aria-describedby attribute on the
input is probably the best way to do this.
This is especially important for inputs which require a specific format (such
as dates)
Affects latest master (using Orca 3.4.2)
