Search and filter forms need labels

Bug description:
In most filter forms, the combobox used to filter does not have an
associated label. Also, in some cases the search box doesn't have a
label. This is confusing for screen reader users since it's unclear
what each of these elements does.
This affects the following pages (latest master)
- Portfolio -> Pages
- Portfolio -> Skins
- Groups -> My groups
- Groups -> Find groups
- Groups -> (group) -> Pages
- Groups -> (group) -> Members
- Inbox
