There are also instances of duplicate accounts from LDAP: eg

NAME u77777771 u7777...@uni.canberra.edu.au LearnOnline UC LDAP 
NAME U7777777  u7777...@uni.canberra.edu.au LearnOnline UC LDAP

So maybe you don't need a mahoodle env to test this one.... (???)

Bug description:
  Mahara 1.8, Moodle 2.5

  Hi Everyone,
  This is really a Mahoodle bug, maybe.

  Scenario someone logs into Mahara via LDAP, an account is created if the user 
is authenticated.
  Same user logs into Moodle later on, then SSO's to Mahara - another account 
is created with a "1" after the student ID.

  This only happens for some people, the other important thing to note
  is that the authentication fallback order is:

  LDAP
  XMLRPC
  Internal

  The bug seems to be that Mahara isn't checking if a user has an
  existing account regardless of which authentication methods was used
  to create the account.
