Links for hiding/showing more content need accessibility info

Bug description:
Some links in Mahara are simply used to toggle the display of more
information.
For example in the inbox or the my notes page.
For a screen reader user clicking on the link gives no feedback that
new information has been displayed (or hidden) as no new page (or new
content) is loaded.
There needs to be some accessible-hidden text that indicates what the
link is actually doing.
