Thank you for the report and sorry for the delay in replying. I don't
think this would be something for us to fix or easily fix.
When a user registers and the admin goes through the pending
registration approval, the user has 24 hours to reply and accept to set
up an account on Mahara.
If he again registers an account and the admin approves that as well,
another approval link is sent. This one is then actually the only
correct one and the previous link gets invalidated and can't be used.
If the user registers twice in a row with the same email address and the
admin didn't already approve the first request, the requests are
actually joined and displayed as one registration to the admin.

Steps to reproduce:
- user asks for joining an institution
- user keeps his session active
- admin validates the request
- user asks again for joining same institution
- admin validates again already validated request => bug
