at line 699 :
699 $elements['actve'] = array(
670 'type' => 'switchbox',
671 'title' => get_string('active', 'auth'),
672 'defaultvalue' => (int) self::$default_config['active'],
673 );
Line 699 should read : $elements['active'] = array(
"active" instead of "actve"
