Annotation block title doesn't pass accessibility

Bug description:
Follow-on from the Bootstrap 4 upgrade.
When you place an annotation onto a page via the SmartEvidence matrix
page, the title is greyed out. The background is too dark and doesn't
pass contrast.
The same goes for the greyed out institution name when you set up a
group.
