Title:
  Allow SAML to create institution if it doesn't exist before adding new
  user to it

Status in Mahara:
  Fix Committed

Bug description:
  Create a site config setting where we can indicate one or more
  institutions that can act as 'template'  SAML auth instances so that
  if a user logs into their IdP and their institution does not exist yet
  on Mahara we can created it and set it up with the correctly
  configured SAML auth
