Did you already upload an image that looked like the thumbnail to your
instance of Mahara in the past and then only cleared out the database
but didn't delete the site files when you reset your instance?

Image thumbnail icons not matching listed file

Bug description:
Browser: Google Chrome
Steps to reproduce:
- Create Image Gallery block
- Add images, some from existing uploads and some that you upload within the
edit window
- Check the image thumbnails on the left of the list of files
Expected:
- Image thumbnails should correspond to the listed file
Actual:
- In some cases, this does not correspond to the actual image
