When registering external apps, currently we are able to add more than
one external app with the same name, same institution and same service
group
Steps to reproduce:
1. Add an external app via the External apps page
2. Add another external app with the same name, same institution and same
service group
Expected outcome:
- Prevented from doing this
Actual outcome:
- Allowed
