Title:
Unlabeled toggle in Group Plan tasks page

Bug description:
Discovered using myportfolio.school.nz
Steps to reproduce:
- As Institution Staff, create a controlled group, and add some members
- Create a plan
- Add a task
- Add a portfolio page to this task
- Log in as a group member
- Go to the group page > Plans > Click on the plan that was created to view
the tasks
- To the right, on the line where the plan is listed, there is a toggle
button with a red dot saying "No".
Expected outcome:
- Above the column where the toggle is, a heading should indicate what
clicking the toggle will do.
Actual outcome:
- There is no heading above this column to indicate what clicking this toggle
will do.
Further steps:
- When I click the toggle, it changes to a green dot, with "Yes", and a green
banner says "The plan and selected tasks were transferred to your personal
portfolio area. If applicable, assignment portfolios were copied as well."
Expected outcome:
- Tasks are found in my portfolio area i.e. Pages and collections
- Pages that were included in the plan are editable by the person who has
added them to their personal portfolio area.
Actual outcome:
- Tasks are found under my Plans page
- Pages included in plan are not editable
