The online users page should make sure users are only displayed once.
Currently users who are in more than one institution can be displayed
twice in the list.

Bug description:
The online users sideblock can get very large (and make the page
ridiculously long) on a busy site.
We should put a limit on the number of online users that get
displayed, then we should have a "see more" link which leads to a new
page that shows the whole list.
