Series of enhancements to auth/saml to enable a user to link their own
account to a simultaneously logged in remoteuser:
- dual login – ssphp, and Mahara session integration for login-login-link
- show login or link to register page
- on login for link - take to link confirmation page
- preserve returnto URI
