Again, I can only repeat that this is trivial: there is already code
invoking the Python hooks, it's enough to just add the possibility to
optionally call an external program instead/together with doing this. I
definitely want to do it, but I don't know when will it happen. Any outside

Sorry, what do you mean by "on a case by case"? Do you really want the
filter to ask you for every message if it should match is case sensitively
or not?
