 Well, it does need the name of the file to store the messages in. If
you're confused by the wizard you could also try cancelling (on the first
page) and going to the folder properties dialog instead.

 The folder name is the name which appears in the tree while the mailbox
name is the name of the physical file. They can be (and often are) the
same, but they also can be different.

 I'm afraid that this was broken in 0.64 which you seem to be using. In
0.64.2 the parent of the new folder will be the folder currently selected
in the tree. Again, you can specify it explicitly in the folder creation
dialog which you get if you cancel the wizard.

 You should press Ctrl while dragging (the cursor should change).
