Mahogany doesn't create $HOME/.M under Windows, it uses another directory
(currently its current one but this is wrong and should be changed). Also,
it doesn't put all default values in ~/.M/config because this would make
for a huge file.

This is not supported under Windows, you can set up the global (i.e. for
all users) config only by creating the values under HKLM registry hive.

It shouldn't be necessary to have $HOME, I think this should be a choice
in the installation program or the initial wizard. If you choose to use a
config file it should simply store its name in the registry and, if this
value is present, use it on the subsequent runs.
