In that case, administrators often explicitly subscribe almost all the users, using some "offline" method to ensure that the users do wish to be subscribed. But when a subscriber address changes, it's not easy for the administrator to change the address reliably. Since the "address change" on the user options page will generate a confirmation message that the subscriber will likely ignore, administrators who don't have access to the clone command may choose to use an unsubscribe/subscribe strategy to get the list up to date. This, of course, risks losing user-configured options, but is an attractive path for those who have struggled and failed to explain confirmation messages.
