Double opt-in refers to what should be the only way you can sign up for a mail server list. That is, the user, as part of the sign-up, enters her email address. A confirmation email is sent to that address and the user is not actually added to the list until the action specified in the email (usually just a reply, but it can be a URL also) is taken.
This eliminates the possibility of false sign-ups - pranks.
