Is the user receiving the unwanted email in the same domain as the where the list is hosted? Have you used the ~mailman/bin/list_members script to see if perhaps the CGI's are misrepresenting your list members perhaps? There isn't really a lot of information in your post, so I'm not sure if I'm asking the right questions.
