It's going to depend a lot on your MTA and how it's configured, and whether or not you are making use of personalization features.
The people who got the message have email addresses throughout the alphabet. Some of the addresses starting with w,f,k received it but then others like e,m, and some other w did not. How do I troubleshoot this and prevent it from happening again.
Even if you're not using personalization (so that Mailman creates the smallest number of messages possible and then hands those off to the MTA), the MTA may well deliver messages to the recipients in parallel. So, some users further down the list might receive the message before others higher, simply because of the difference in how fast/overloaded the recipient mail servers are, etc....
Several messages sent later in the day delivered fine without any problems.
Without knowing extensive details of your configuration, it's going to be impossible to debug this process. Indeed, even on a mail server where I have full access to all available information, I can't always tell you what went wrong or what you should do to ensure that doesn't happen again.
