Okay, stupid question: where is the list normally set to be "moderated?"

Yes/No on the General Options page, but this is probably not it because owners don't get individual notifications fo messages held for "emergency". You need to look at Mailman's "vette" log or at the held message in the admindb interface or at the list owner notification and see what the reason is for the message being held. If the reason is "post to moderated list" the post is from a member whose 'moderate' flag ('mod' on the admin Membership List page) is set.
