Just had a very odd occurrence. I have a handful of users on moderation, for various reasons.
I received a single moderation notice for one of them, and after reading the user's text, went to the web site to approve it. There was only one message there for approval. I approved the message, and it was sent -- but here's the glitch: it got sent along with another separate message from him. Then in my next mail check there was the second moderation request, which never showed up for approval on the web site. Somehow, Mailman got confused and approved both messages. I looked, and his moderation flag is still on.
