Any chance that the message in question had a From: address in a domain that publishes a restrictive DMARC policy? That can cause bounces all over the place. The usual suspects here are aol.com and yahoo.com, but a lot of companies that believe (usually wrongly) that they have a problem that DMARC can solve set DMARC policies, too.
