I've seen this myself on very rare occasions.
I'm not really sure what triggers it and it's
tricky to debug. (I'm not even sure it's a
MailMate only issue.)

I haven't seen it in any other in-program upgrade
so far and most programs use that mechanism
nowadays. At first glance, it looks like the
script that does the upgrade failed to remove the
old icon from the dock. If that happens only
rarely, it could be a timing issue.
