You've invented a non-existent rule. I hope no one has made the blunder
of enshrining that non-rule in code or teaching it as fact.
No mention of such a magic word exists in RFC1869 (which defines the
SMTP extension model,) RFC2821, or RFC5321. All three include example
ESMTP chats without ESMTP in the opening 220, followed by a EHLO.
