Pretty sure that form is for telling Google that they made a mistake in classifying your site as malicious, not for requesting a rescan. My advice would be to sign up to the Google Search Console (a.k.a. Webmaster Tools) and request a rescan from there after you are sure that you have addressed any issues displayed there.
