It's a matter of playing the odds.  Also keep in mind that some TLDs are a lot
bigger than others.  CAM only has 36,000 names which is pretty small so if 
you're
seeing a lot of spam from a small TLD, that usually means they have a cheap bulk
purchase promotion going on so block away.

Depends on the country.  Some small countries rent out their ccTLDs and the
quality control is highly variable.  The .TO TLD is fine, but I don't think
I have ever seen a message from .WS or .TK that I wanted and I doubt I
ever will.
