Here's how the message counter/notifications work:
- Gmail checks for new email automatically. As soon as it detects new
email, Mailplane will access the RSS feed for your inbox to get the
message subjects and show notifications.
- For reasons unknown, Gmail can stop checking for email or it takes
unusual long time - in this case Mailplane will still read the RSS
feed and show you new email notifications. This is why your Gmail
isn't up-to-date.
- Mailplane will access the RSS feed at startup and may be faster than
Gmail in updating its inbox. My guess is that you'd see the unread
messages after some delay (without clicking the "Refresh" link).
- If you use Google Gears for "Offline Gmail", this can delay the
Gmail message checking even further.

Unfortunately, Mailplane cannot change the way Gmail works - so
there's currently no solution or than using the Refresh link or the
Mail > Refresh ⇧⌘R keyboard shortcut. For some users, creating a new
Gmail session solves the issue.

I am currently considering doing an "auto-refresh" when Mailplane
detects new messages and Gmail "forgets" to update.
