Once per user, not once per universe.  At least potentially.

Yes, I know that many DOS/Windows users tend to download precompiled
binaries.  But imagine that, for some reason, one needs to rebuild
everything from scratch: one would then need to start from a few basic
tools, and Make is one of them.
