1. I am using version 1.0.1. Therefore when I put <noPreload> in, it doesn't
make a difference. I will need to upgrade.

2. I have grouped the layers through geoserver.
http://docs.codehaus.org/display/GEOSDOC/What+is+the+Layer+Group+option. I
then made the ranges non-overlapping by using the elements
<MaxScaleDenominator> & <MinScaleDenominator>. I then referenced the group
in the context file. This seems to work.

Which version of MapBuilder do you use?  On version 1.5-alpha1, there's
an option for MapPane:
  <noPreload>true</noPreload>
that only loads the visible layers.

Not that I know of.

There are two: MapScaleBar and MapScaleText.  For an example of
