Although it is not particularly user friendly the absolute foolproof way
to get this right is to decide on the width of frame you want in your
layout and then set this. Next multiply this width by your desired
scale. Now switch focus to your Map Window and set the map to the
calculated width. Now switch back to the Layout and you are ready to
print.
For example if your frame is six inches wide and you want a scale of
three miles to the inch - set the frame to six inches - switch to map
space(mapper) and set the width to 18 miles - switch back to
paper(layout window) and print.
Example two. You want a frame of 180 mm and a scale of 1:1250. Set the
frame width to 180 mm. Switch to map window -set width to 180 * 1250 mm
= 180 * 1.25 metres = 0.225 km. Now switch back to paper and print.
Its not user friendly as I think it is reasonable to expect to set scale
while in Layout rather than switching to Map - but it works every time.
If you have MapBasic - this can of course be programmed.

PS if you set the scale in the layout window this gets the scale right
but it achieves this by using the existing map width and changes the
frame width instead!
