In the latest release of Vertical Mapper (version 2.5) the X axis units for
the Cross Section Tool can be set to a variety of distance units including
kilometres.  This is done in the Preferences dialogue (Vertical Mapper >
Preferences).  In earlier versions the X axis was determined by the distance
units of the grid's projection.

If your version is older than than version 2.5 then try creating a custom
projection with the unit identifier in the projection string set to
kilometres (m=7, km=1).

For example change this:
"UTM Zone 18 (NAD 83)\p26918", 8, 74, 7, -75, 0, 0.9996, 500000, 0

To this:
"Custom Projection", 8, 74, 1, -75, 0, 0.9996, 500000, 0
