1.  It could be that you are attempting to digitize off of more than one
image at a time in the same map window.

The objects you are digitizing will be accurate with only one of the images.

Digitizing from multiple images is advisable only if you're using tiles
of digital orthophotography already rectified to a particular projection.

2.  Make sure that the table you are drawing objects into is using the SAME
projection that the raster is.   On-the-fly projection conversion is
notoriously
innacurate, especially if  two projections use different datums.
