Do you already have a program (database) with that information in it? If so,
there is an excellent program called GeoBasemap MapLinker (by an Australian
company called Exa-Min) which allows you to hotlink to either standard or
customised applications based on a specific ID. Therefore, you can click on
a map object and be shown the specific database form which corresponds to
the object (you can also hotlink to pictures, documents, spreadsheets etc if
you need to).

I wish that a click on a object in my mapinfo map give output a form or
