To create custom symbols make sure that your symbol is has 256k
colour depth 
or less. 
If it is set to 24-bit colour  then the symbol will not show. 
The symbol has to 32x32 pixels or less and needs to be in the same
directory 
as 
the other custom symbols.
Probably :- C:\Program Files\MapInfo\Professional\CUSTSYMB

I think this has changed in version 6.5 though
