Chris, I'm not sure that I have a solution for the page count shrinkage problem you're experiencing, but here are a couple of suggestions for the other issues :
To print to the edge of the page from Acrobat, you need to check the Display Page to Edge box in the General Preferences dialog. There are several ways to append or insert existing PDF files. One simple method to merge several PDF files is to open the first file and then select Insert Pages from the Document menu. You will be prompted to locate the file which you want to append and once selected, you can then specify in the Insert dialog exactly where you wish to place the second file, most probably at the end of the first. Repeat as necessary for other PDF files. I'm still using Acrobat 4.05a but the above holds good for 5.x as well.
