You probably have one registered point that has the wrong coords. Look at
the error in pixels ("Table>Raster>Modify Image Registration") and delete
the one with the most. You may need to add another one before being able to
exit. Also, you will probably need to create a new mapper since MI seems to
remember the incorrect projection. If it gets out of control, delete them
all and start again!
Do you know of the little publicised ability to snap to say a street
intersection while the Modify Registration window is still open? Very
useful. "Table>Raster>Select Control Point from Map"

Dear All
I am trying to open a raster image in MapInfo, regsiter it and then
overlay census boundaries on top of the image. I manage to register
the map and it appears to be in the correct location but when I
overlay the boundaries they become totally distorted and "elongated".
Has anyone had similar experiences?
