I worked on this one and a half years ago. There was no direct support of map rotation in mapnik then, and I had to adopt a rather tedious manual way, by rotation of all data sources around the same point, and then rendering the map with the resulting fake coordinates. Some coarse details are given at the bottom of http://www.ancalime.de/rotated.html . Rotation of vector data in PostGIS was easy, as this has support for affine transformations. The most difficult part turned out to be the rotation of raster data, as not all converted data could be read in by mapnik again. If you are interested, I could discuss this in more detail and also provide code snippets in ruby (this is just part of a larger system, so you would need to modify and integrate it yourself).
