Follow-up Comment #2, bug #37718 (project maposmatic): This happens if the Python bindings for Mapnik haven't been compiled with Cairo support.
With the bundled bindings in Mapnik v2.x this happens if python-cairo-dev was not installed at the time of building Mapnik.
