Hello,

My quick thoughts:

- remove all projection objects
- use the commandline gdalinfo utility to get the extents of your image (Lower Left and Upper Right coordinates) and use those values in the EXTENT parameter - test your mapfile with the shp2img utility (http://www.mapserver.org/utilities/shp2img.html)
