Frank - gd.h was missing from this location but present in
/usr/local/include
I ran the gd make install again (or maybe I had overlooked to run it the
first time?) and the gd.h appeared in /usr/include/ and mapserver
configure script has run OK

If not, you are most likely missing the GD -devel package.
