I'm facing the exact same problem with MS4W 3.0.3 on Windows Server 2008 RC2 64bits. I tried your solution using the MSVC2008 (Win64) -release release-1500-x64-gdal-1-8-1-mapserver-6-0-1 package but it didn't solve the issue in my case.
