It could be that you updated a shapefile without updating the spatial 
index.   Check your data folder and look for a qix file with the same 
name as your shapefile.  If the date is older than your shapefile you 
will either need to delete it, or update it using shptree 
http://mapserver.org/utilities/shptree.html.
