I did not follow the whole thread, so probably this idea is not useful:
I made the experience that with "shp2img" you are getting error messages
easier, because they are displayed directly on screen. Perhaps this can
show some new information.

P.S. I struggled a long time with cascading WMS before I understood that my
test server was behind a proxy. I suppose this is not the case with you.
