DESCRIPTION:
Change Item::name, Item::org_name to LEX_STRING objects (from char*)
This allows us to remove Item::max_length and some calls to strlen(Item->name).
Reasons:
- Simpler and faster code
- Allow us to remove calls to strlen()
- Fixes some known bugs where Item->max_length and Item::name is not kept in
sync.
